Introduction to Accident Investigation

Course Description

Overview

This course provides an introduction to basic accident investigation procedures and describes accident analysis techniques. The goal of the course is to help participants gain the basic skills necessary to conduct an effective accident investigation at their workplace.

Topics include the primary reasons for conducting an accident investigation, employer responsibilities related to workplace accident investigations, and a six step accident investigation procedure. The course is set up as a facilitated, interactive training session focusing on class discussion and group activities.

Intended Audience

The target audience is the  employer, manager, employee or employee representative who, as part of a firm?s safety and health system, would be involved in conducting accident and/or near miss investigations.

Reasons for Development

Workplace accidents occur each and every day all across the country. Each year the Bureau of Labor Statistics ( http://www.bls.gov/bls/safety.htm ) publishes a statistical summary of injuries and illnesses that emphasizes this fact.

The failure of people, equipment, supplies, or surroundings to behave or react as expected causes most of the accidents. Accident investigations determine how and why these failures occur. By using the information gained through an investigation, a similar or perhaps more disastrous accident may be prevented.  Accident investigations must be conducted with accident prevention in mind. Investigations are NOT to place blame.
